In order to participate in scouting activities, all scouts are required to have a medical form on file with their den leader.  Below you will find the BSA medical form Part A and Part B.  Please fill out the necessary forms and attach a copy of you son's medical insurance card and return to your den leader.   The medical forms DO NOT have to be signed by a physician.  You may also attach a copy of your son's most recent physical.
