The troop must obtain BSA Health Forms and ensure the troop has non-expired forms at each Troop Campout for safety. New Health Forms (Parts A and B--Information and Parental Consent) are due at the beginning of each Scout year. In accordance with BSA policy, Scouts must have current health forms (A&B) on file to attend monthly campouts.
Due Dates for Troop Health Forms:
Parts A & B: September 2014
Part C: June 1, 2015
Forms you need:
1. TROOP MONTHLY CAMPOUTS
All Scouts and Camping Adults by October Campout:
Parts A and B
2. SUMMER CAMP
All Scouts and adults (even if only 1 night) in addition to A&B you need by
June 1, 2015: Part C (If not going to Summer Camp, no need to turn in Part C to me)
3. HIGH ADVENTURE CAMP (Philmont, Northern Tier, Florida Sea Base, Summit Bechtel)
All Scouts and Adults, ensure the doctor certifies that he/she has reviewed the supplemental risk advisory for the high adventure program in which you are participating. For Sea Base, if you will be SCUBA diving, you also need the PADI medical form.
High adventure camp attendees:
YOU NEED A COMPLETE COPY OF YOUR OWN HEALTH FORMS. That means if you are going to Sea Base, you must have Forms A, B, C, & PADI on your person.

How to Complete the Forms:
1. The forms are fillable .pdf files. Type your answers into all applicable blanks. Make sure you have a tetanus date.

2. Provide a copy of your insurance card by overlaying it on blank area at bottom of page 2 of Part B and make a copy (best); or tape a copy of the insurance card to this area on Part B.
3. Parent/Guardian sign and date form on Part B.
4. Turn in 1 simple copy to me (A/B & C). Keep a copy for yourself. If signed, I can accept email versions. Otherwise, you can give the forms to me in hard copy signed.
Sports Physicals: If you are getting a sports physical, you can ask the doctor to complete your BSA physical form (Part C for summer camp)(Parts C with high adventure acknolwedgement by doctor on Part C) at the same time. You should not need to get two physicals. Plan ahead and take the BSA form with you to your sports physical or take it to your doctor at your next appointment.
BSA physicals are good for one year. Avoid having your physical expire in June or July during summer camp season by timing the anniversary of your physical to occur in a different month other than June or July!
However you do it, make sure you get your physical exam done BEFORE June 1, 2015!
