Medical Form (typeable pdf) This form needs to be completed each year and is good for only 1 year.  It is usually updated before summer camp.  Tigers, Wolves and Bears usually only need Parts A and B.  Webelos going to summer camp need Part C (signed by a doctor) because the event lasts more than 72 consecutive hours.
